As of April 11, 2022 North Dayton ObGyn has closed. ObGyn care will continue to be offered at the following Premier Physician Network location:

There is no need to transfer your records if you continue to see a Premier Physician Network provider. If a copy of your records is needed for a provider outside our network, a Records Request Authorization Form should be faxed to (937) 277-9035 or you can use our convenient, on-line portal at www.premierhealth.com/RequestMedRecords.
