Appointment/Procedure Preparation
To schedule an appointment, please call the office at (937) 208-6920.
You will be given specific instructions at the time your appointment is made, as preparation may vary by physician and type of visit.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Bring
If you are a new patient, please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). Established patients should bring new insurance card(s) when applicable.
All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.
After Your Visit or Procedure
After your visit, you will receive instructions for making a follow-up appointment for monitoring of chronic health conditions. Your “after-visit summary” will indicate when you should return, orders and/or instructions from your visit. If tests have been ordered, you will receive a follow-up phone call, a letter with your results or a MyChart email.
