Appointment/Procedure Preparation
To schedule an appointment with any of our physicians, please contact our office at (937) 208-7600(937) 208-7600. We will provide any special instructions at the time of your call.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
All new patients are required to bring a picture ID on the first visit. Please bring your insurance card to each visit.
After Your Visit or Procedure
You will be contacted by phone or e-mail or check your MyChart messages within 10 days following completion of a lab or test. If the results are urgent we will contact you by phone.
Billing and Insurance

Beavercreek Family Physicians does not accept workers’ compensation cases.
