MyChart Proxy Access
MyChart® proxy access allows someone to access another person's MyChart account. This feature is commonly used by parents of minor children and by spouses, caregivers, and guardians to assist in managing the care of a loved one.

Important notes/considerations:
- To be a proxy, you must have your own Premier Health MyChart account.
- Proxy access may include full or limited access to a patient's medical information, depending on the access level granted.
- Adult patients can cancel proxy access at any time.
- Proxy access should only be granted to individuals you trust.
- All proxy access ends when the patient is documented as deceased in their medical record.
Proxy Access for a Child
Parents or guardians may request access to a child's medical record through MyChart until the child turns 18. Proxy access is linked to the parent or guardian's MyChart account and does not require a separate username or password.
To request access, submit the completed Parent or Guardian Request for Child Proxy Access Form (PDF), using one of the available submission methods. If you are not the child's biological parent, you must also provide documentation of legal guardianship.
Cancel Proxy Access for a Child
Proxy access automatically ends when the patient turns 18. At that time, the patient gains full control of their MyChart account and may choose whether to share access with a parent or guardian.
Renew Proxy Access for a Child
No renewal is required. Proxy access remains active until the child's 18th birthday.

Proxy Access for an Adult (18+)
Adult patients can grant proxy access through the Sharing Hub in MyChart.
Access Levels
MyChart allows you to select from the following access levels when inviting someone to be your proxy.
- Full Access: View clinical information, messages, and scheduling information.
- Clinical Only: View clinical information only. No access to messages or scheduling.
- Scheduling and Messaging Only: View messages and scheduling information only. No access to clinical information.
Invite a Proxy Through MyChart
- Go to the Sharing Hub in MyChart. You will be prompted to log in if you aren't already.
- Select Family member, close friend, or caretaker.
- Select Manage ongoing access to your MyChart account.
- Select Invite friends or family.
- Enter the required information.
- Choose an access level.
- Review and accept the Terms and Conditions.
- Select Send Invite.
The invited person will receive an email invitation. If they do not already have a Premier Health MyChart account, they will be prompted to create one. They will also be asked to verify your identity using your date of birth.
Grant Proxy Access Without a MyChart Account
If the patient doesn't have a MyChart account, proxy access can be requested by submitting a completed Adult Proxy Form (18+) (PDF) using one of the available submission methods.
Cancel Adult Proxy Access
The patient can cancel proxy access at any time through the Sharing Hub in MyChart.
You may also submit a written request using one of the available submission methods.
Renew Adult Proxy Access
Adult proxy access expires two years from the date it is granted.
Once proxy access has expired, the patient can renew proxy access through the Sharing Hub in MyChart .
The proxy can request access to be renewed by submitting a completed Adult Proxy Form (18+) (PDF) using one of the available submission methods.
Guardianship Proxy Access
Guardianship proxy access may be available when a patient can't make or communicate healthcare decisions independently. Examples may include dementia, developmental disabilities, or other conditions that affect decision-making capacity.
The person requesting access must submit the completed Adult Proxy Form (18+) (PDF), along with documentation of legal guardianship, using one of the available submission methods.
Guardianship proxy access can't be granted by the patient through MyChart.
Cancel Guardianship Proxy Access
Legal documentation is required to change or cancel guardianship proxy access.
How to Submit Proxy Access Requests
Completed proxy access forms, any required supporting documentation, and requests to cancel adult proxy access may be submitted using one of the following methods:

Once a request is received, it may take up to three business days to appear in MyChart.
