Appointment/Procedure Preparation
Women’s Health Specialists & Midwives of Dayton is currently accepting new patients. To schedule an appointment, please call (937) 890-6644.
Any special instructions regarding your appointment will be discussed with you during the initial phone conversation.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
If you are a new patient, please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). Established patients should bring new insurance card(s) when applicable.
All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.
After Your Visit or Procedure
After your visit, you will receive instructions for making a follow-up appointment. If needed, you will also receive information about prescriptions, blood work, or other orders.
You also will be given the opportunity to sign up for MyChart – an electronic medical record which allows you to review your medical information online and communicate with the team at Women’s Health Specialists & Midwives of Dayton through the secure Website. MyChart also allows you to schedule appointments, check your current and past lab results and ask other medical questions. If tests have been ordered, you will receive a follow-up phone call or a letter with your results.
Billing and Insurance
View the list of the major insurance plans accepted by most Premier Physician Network offices. Women's Health Specialists & Midwives of Dayton accepts most of these plans.

Hospital Pre-Registration
We encourage all mothers-to-be to pre-register for their birthing experiences. Taking the time to complete paperwork before you arrive to deliver your baby will make your check-in quicker, allowing us to have you in your room in less time. Pre-registration can be completed at your hospital's registration office or online.
