Planning Care After Hospital Discharge


Before you leave the hospital, your doctor or nurse will talk with you about the possibility of follow-up care in your home, if needed.

What Does Follow-Up Care Include?

Before you leave the hospital, if your doctor thinks you need follow-up care in your home, a visit by a nurse will be arranged. During the nurse’s first visit with you in your home, she will meet with you and your family or caregiver to discuss what kind of assistance you’ll need. These health care professionals are all part of the Fidelity Health Care team and are ready to help you at your home. You, your caregiver, and your nurse, along with input from your doctor, will determine your plan of care and if insurance coverage is available.

What If I Decide Later That I Need In-Home Care?

If it’s decided that you won’t need help at home, but then you later realize that you do, that help can be arranged in one of several ways:

Call us at the numbers below to discuss the care options available to you and to determine whether your insurance will pay for it. Because in-home care requires a doctor’s order, we can help obtain that order. If you don’t have a health care provider, we can help you get one.

Two days after discharge, a Premier representative will phone you to ask how you’re doing. If you decide you need help of any kind, it can be arranged then.

Two weeks after discharge a Premier representative will phone you again to be certain your health is improving as expected. In-home care can be arranged at that time if you decide it would be beneficial.
