Congestive Heart Failure
If you have congestive heart failure (CHF), your heart doesn’t pump blood as efficiently as it did when you were younger. CHF can make you short of breath, leave you feeling tired, cause swelling in your legs and feet, and create other health problems for you.
Following your doctor’s orders, Fidelity Health Care provides personal, expert care to treat your CHF and help you manage your symptoms — in the comfort and privacy of your home. We’ll make sure you understand your condition and the treatment we’re providing, and we’ll help you learn about and manage the medicines you’re taking. We will also train family members or caregivers who are helping you at home.
Is In-Home CHF Care Right for You?
If you have CHF, you could benefit from in-home care if:
- You have had a recent hospital visit related to your heart condition and need help transitioning back to your home
- You have other health problems along with your CHF, such as diabetes or chronic obstructive pulmonary disease (COPD), or if you need oxygen
- You are home-bound by your health problems and have difficulty getting out to see a doctor
Who Will Come to Your Home?
Your CHF treatment will be managed by a nurse working closely with your doctor. Your treatment team could also include one or more of the following professionals:
During the visits, the nurse will assess your physical condition and discuss any symptoms that might be bothering you. The nurse will also talk with you and your family members or other caregivers about your medications, possible side effects to watch for, and any other concerns or questions you might have.
How Long Will You Need to Have In-Home CHF Care?
Every patient is different, and the length of time you’ll need care depends on several things, including how severe your symptoms are, how well you’re responding to treatment, and your doctor’s orders.
