Our goal is to streamline care for you or your loved one and to help ensure safe and successful management of blood-thinning (anticoagulation) therapy.

Common reasons for taking blood-thinning (anticoagulation) therapy include:
- Following a heart attack
- Stroke
- Blood clotting disorders
- Artificial heart valves
- Atrial fibrillation (irregular heartbeat)
- Treatment or prevention of deep vein thrombosis (blood clot in a large vein, most commonly in the leg) or pulmonary embolism (blood clot in the lungs)
From our pharmacist-managed medication therapy management services, you or your loved one receive speedy, comprehensive care that includes:
- For those on warfarin (Coumadin):
- Administration of finger stick blood test
- Rapid INR results
- Any needed adjustments to your medication
- o Education on your medications and food interactions that you need to know about to ensure your safety
- For those on other anticoagulation medicines (such as Eliquis, Pradaxa, Xarelto, and Savaysa):
- Proper dosing and monitoring
- If you’re having surgery or a procedure, assistance to you and your doctor on the best way to manage your medication before and afterward.
- Medication refills ordered as needed
- Discussion and counseling with a supportive care team that gets to know your individual needs and works with you create a safe and effective plan of care
A health care provider must refer you or your loved one for medication therapy management services. Talk with your doctor, or contact us and we’ll work with you to get a referral. Our experienced pharmacy team will keep your doctor informed about your care. You or your loved one can receive care from our team as long as you are taking blood-thinning (anticoagulation) medication.
