Our goal is to streamline care for you or your loved one and to help ensure safe and successful management of blood-thinning (anticoagulation) therapy.

Common reasons for taking blood-thinning (anticoagulation) therapy include:

For those on warfarin (Coumadin):
- Administration of finger stick blood test
- Rapid INR results
- Any needed adjustments to your medication
- o Education on your medications and food interactions that you need to know about to ensure your safety
For those on other anticoagulation medicines (such as Eliquis, Pradaxa, Xarelto, and Savaysa):
- Proper dosing and monitoring
- If you’re having surgery or a procedure, assistance to you and your doctor on the best way to manage your medication before and afterward.

Our experienced pharmacy team will keep your doctor informed about your care. You or your loved one can receive care from our team as long as you are taking blood-thinning (anticoagulation) medication.
