Community Health Improvement
At Premier Health, we’ve fully embraced the goal of building a healthy community. We are committed to supporting initiatives that will positively affect the people we serve.
Every three years, nonprofit hospitals complete a Community Health Needs Assessment (CHNA), as required by law. Below are links to our CHNAs and other Community Health Improvement reports.

2013 Assessment and Improvement Plans
An essential part of knowing how we can improve the health of our community is to understand its unique health issues. To that end, Premier Health was part of a collaboration in 2013 with the Greater Dayton Area Hospital Association and hospitals throughout Southwest Ohio to conduct a regional Community Health Needs Assessment. This assessment assisted us in identifying areas of opportunity to improve community health.
This report shares our plan for improving health in the identified priority areas in our region.
