Atrium Medical Center
Outpatient radiology, respiratory, physical medicine, lab, and EEG: Register in Outpatient Diagnostics, level one, atrium, across from Atrium Café
Surgical admissions: Register on the second floor of Atrium
Family Birth Center: Register on the third floor of Atrium.
Outpatient cardiology patients: Register on the second floor of the Professional Building, which connects directly to Atrium.
Patients entering the Wilbur and Mary Jean Cohen Women’s Center: Register on the fourth floor of the Professional Building, which connects directly to Atrium.
Patients reporting for cardiology diagnostics: Report to Patient Access in the main lobby.
Direct admissions from the physician’s office: Will be registered at the patient’s bedside on the nursing unit.
Emergency Trauma Center is located on the east side of Atrium. Follow Campus Loop Road to parking lot L. Patient registration will be conducted at bedside in the Emergency Trauma Center.
Miami Valley Hospital
Cardiology patients should report to the cardiovascular registration area located in the southeast addition of the main lobby.
Unless instructed otherwise, all outpatients and inpatients should report to the Admitting/Outpatient Registration area on the first floor of the hospital, zone A. Please check in at the main access front desk. The patient access representative will enter the patient’s name in access tracking, hand the patient a pager, and have him or her take a seat in the waiting room until the pager is activated.
Patients are registered in order of arrival. Any patient requesting a wheelchair or assistance will be helped by hospital escorts. Once the pager is activated, the patient will be directed or escorted to the registrar’s booth where the registrar will collect or update patient demographic information, have the patient sign required forms, scan insurance cards and other documents, and collect co-pays.
Patients will then be directed or escorted to the ancillary testing area or the unit where they will be admitted.
Miami Valley Hospital North
Registration for outpatient medical imaging/diagnostic testing is located on the first floor in the front half of the building. Mammography registration is located on the first floor in the rear half of the building.
Registration for inpatient services/surgery is in the basement level of Building 3 via the inpatient entrance.
Miami Valley Hospital South
Registration for outpatient medical imaging/diagnostic testing is located on the first floor. Upon entering the main entrance, walk straight down the hall past the information booth to the outpatient medical imaging registration desk.
Mammography registration is located on the first floor in the Breast Center. Upon entering the main entrance, turn right down the hall until you arrive at the elevators on your right (across from Kobricks Coffee Shop). Take elevators to the second floor and make an immediate left off the elevators. Breast Center entrance will be on the left.
Registration for general surgery is located on the second floor. Upon entering the main entrance, take elevators located on the left to the second floor. Make an immediate left off the elevators. The registration area will be located straight ahead.
Registration for Joint and Spine Center is located on the second floor. Follow Miami Valley Drive (back of facility) to Parking Lot D and enter directly through the Joint and Spine Center building at Miami Valley Hospital South. Take the elevator or escalator to the right of the information desk to the second floor. The registration area will be located straight ahead.
Upper Valley Medical Center
Patients who come to UVMC for medical imaging should report to the Outpatient Services Registration desk in the main lobby. Scheduled procedures including CT, MRI, nuclear medicine, ultrasound, mammography, and fluoroscopy should arrive as directed by Central Scheduling. Walk-in appointments for other imaging procedures or X-rays are welcome. Hours of operation for medical imaging are Monday - Friday, 6 a.m. - 9:30 p.m.; Saturday, 7 a.m. - 3:30 p.m.; Sunday, 7 a.m. - 3 p.m.
Patients coming for EKGs and laboratory tests should also report to the Outpatient Services Registration desk. These are mostly walk-in appointments and are performed Monday - Friday, 6:30 a.m.- 6:00 p.m., and Saturday, 7 a.m. - noon.
Patients who come to UVMC for surgery or pre-admission testing (PAT) should report to the Surgery Welcome Center on the second floor. Registrar hours are 5:30 a.m. - 2 p.m. After 9:30 p.m. on weekdays or 3:30 p.m. on weekends, patients are asked to enter through the Emergency Department and check in at the ED front desk.
