Follow-Up Care

When you prepare to leave the hospital after a traumatic injury, a nurse or social worker meets with you and your family about arranging support and ongoing care.

The services you need depend on your condition and can include:

  • Special equipment
  • Nursing care
  • Continued care in another facility
  • Creating a family schedule of care

We work with your health insurance company to determine benefits and preferred providers for care and equipment. If you do not have health insurance, our social workers or financial counselors can help you determine where to apply for assistance.

Levels Of Care In the Community

As you continue your recovery, you may need additional care, including these options:

  • Long-term acute care hospital. If you are able to tolerate three hours or more of therapies each day, you may qualify for placement in a long-term acute care hospital.
  • Skilled nursing facility. If you need continued daily physical, occupational and/or speech therapies, you may benefit from a short stay at a skilled nursing facility. This care is available at many local nursing homes and must be arranged by your case manager or social worker.
  • Home with home care. Your nurse or social worker will arrange, when appropriate, for home nursing or therapy prior to discharge. You will receive the name and contact information for the agency you choose to provide the services you need.
  • Home with outpatient care. If you need outpatient care (physical or occupational therapy, for example) you will get a prescription when you leave the hospital. You will need to make an appointment with a clinic near your home.
  • Home with no home care. In many cases, you will not need specialized home care from a nurse or therapist. Instead, you are released to the care of family with recommendations for following-up with your doctor. You will make your own appointment with the doctor’s office.

Ongoing Recovery Support

At Premier Health we provide additional recovery and rehabilitation services to help you get back to your regular activities after a traumatic injury.

Rehabilitation Institute of Ohio at Miami Valley Hospital. The Rehabilitation Institute of Ohio provides comprehensive services and programs for trauma patients, including injuries to the spine, brain, and musculoskeletal injuries. Certified by the Commission on Accreditation of Rehabilitation Facilities (CARF),the institute helps patients gain strength, relearn skills and manage pain.

Rehabilitation Center at Atrium Medical Center. The Rehabilitation Center is an inpatient rehabilitation facility with specialized, multidisciplinary teams that work with trauma patients for improved outcomes. The center is accredited by The Joint Commission and CARF for Inpatient Rehabilitation Programs – Hospital (Adult) and Stroke Specialty Programs. We provide complete inpatient rehabilitation services for people who have functional disabilities.

Trauma Outpatient Clinic. After you leave the hospital, the Trauma Outpatient Clinic provides follow-up care and oversees your ongoing recovery. Advanced practice providers work with trauma surgeons to manage your progress, wounds, and any acute pain. The clinic also coordinates any ongoing outpatient therapy, medical imaging, and lab tests you need.
