Nutrition Counseling Order

Premier Health offers outpatient nutrition counseling with a registered licensed dietitian to help patients learn how to manage chronic medical conditions.

You can refer your patients to Premier Health outpatient nutrition counseling, or medical nutrition therapy, in two convenient ways:

• If you are on the EPIC system: Submit a physician order through EPIC, and a scheduler from Central Scheduling will call your patient to schedule the appointment.

• If you are not on the EPIC system: Fill out a copy of the paper physician order. Fax the completed physician order to Central Scheduling at (937) 641-2336 (937) 641-2336. After receiving your order, Central Scheduling will call your patient to schedule the appointment.
  

A physician referral for outpatient medical nutrition therapy must include patient name, patient date of birth, diagnosis or diagnoses, appropriate labs if applicable, diet, and dated physician signature (stamps are not accepted). Physician referrals are acceptable for 12 months after the signing date.
